Mandated school physicals are required in first, sixth and eleventh grades. Exams will be provided, free of charge during the school year.
If you prefer your child’s healthcare provider to do the exam, private physical forms may be obtained on the school nurse website or by contacting your child's school nurse.
Mandated school dental screenings are required in grades K, 3, and 7. Screening exams will be provided, free of charge, during the school year. You may obtain a private dental form on the school nurse website, or contact your child's school nurse if you would like your child’s dental exam to be done by their dentist.
Mandated height and weight screenings are done on students in grades K-12.
Mandated school vision screenings are required in grades K-12. A letter will be mailed home to inform the parent if their child has not passed the screening and needs to be referred for follow up with an eye care specialist.
Hearing screenings are performed on students in grades K,1,2,3,7, and 11. Referrals are mailed home to parents for follow up with their primary care physicians if the student had failed two screenings that year.
Mandated scoliosis exams are performed on students in grades 6 and 7. Referrals will be mailed home for any student that requires follow up with their primary care physician.
